New Passport – Form DS-11

You must use this form if you meet all of the following criteria:

• You have never had a U.S. passport; or
• You are younger than 16 years old; or
• Your passport was lost or stolen (in which case also complete Form DS-64)

Passport Renewal (Form DS-82)

You must use this form if you meet all of the following criteria:

• Your prior passport was issued when you were 16 or older;
• Your prior passport was issued within the last 15 years;
• Your prior passport is in your possession

Lost, Stolen, Damaged Passport (Form DS-64)

If your passport is lost, stolen or damaged you must complete Form DS-11 and Form DS-64.

• Your passport has been lost or stolen;
• Your passport is damaged but still valid

Child Passport (Form DS-3053)

If your child is younger than 15 years of age, use this form. Use this form if:

• One of the legal guardians or parents cannot be present at the acceptance agent.

Passport Name Change (Form DS-5504)

If you’ve changed your legal name by marriage, divorce or court order,

you can apply for a passport name change under these conditions:

• You’ve never had a U.S. passport; or
• You’re younger than 16 years old; or
• Your passport was lost or stolen (in which case also complete Form DS-64)

Passport Additional Pages (Form DS-4085)

Use this form if you need additional pages for stamps or travel visas.
